Location: Brooklyn, NY

Schedule: Monday through Friday, 9:00 AM – 5:00 PM

Description
A healthcare organization is seeking a motivated and empathetic Patient Case Manager to focus on patient retention and engagement. This role is ideal for someone who is persuasive, solutions-oriented, and skilled at building trust while resolving concerns.
The ideal candidate is confident communicating with patients, able to de-escalate situations, and driven to meet performance targets.
What You’ll Do
• Reach out to patients who have left or are considering leaving services
• Re-engage and persuade patients to continue or return to care
• Address concerns, resolve issues, and de-escalate conflicts
• Coordinate with internal teams to restart or adjust patient services
• Maintain consistent follow-up and communication with patients
• Achieve weekly retention and performance targets
