Position Overview
An Early Intervention program is seeking organized, compassionate, and motivated Service Coordinators to assist families in accessing and coordinating services for their children.
This position can be performed remotely and requires strong communication, organization, and follow-up skills. The Service Coordinator will work closely with families, providers, and other professionals to help ensure children receive appropriate Early Intervention services.
Key Responsibilities
Serve as a primary point of contact for families participating in Early Intervention services.
Help families understand and navigate available services and resources.
Coordinate services between families, providers, therapists, and other professionals.
Maintain regular communication with families regarding services and progress.
Assist with scheduling and coordinating evaluations, meetings, and services.
Follow up with providers regarding service delivery and outstanding matters.
Maintain accurate and organized case documentation.
Complete required reports, notes, and other program documentation.
Monitor cases and ensure required follow-ups are completed in a timely manner.
Assist families with accessing appropriate community and program resources.
Participate in meetings and collaborate with other members of the child's service team.
Maintain confidentiality and follow applicable program requirements and procedures.
Qualifications
Bachelor's degree required.
Bilingual required.
Strong written and verbal communication skills.
Excellent organizational and time-management abilities.
Strong follow-up skills and attention to detail.
Comfortable communicating with families and service providers.
Ability to manage multiple cases and responsibilities simultaneously.
Comfortable working independently in a remote environment.
Proficiency with computers, email, and electronic documentation systems.
Previous experience in Early Intervention, service coordination, case management, education, healthcare, social services, or a related setting is a plus.
