Position Overview
A growing healthcare organization is seeking an organized and proactive Case Manager to manage a caseload of patients and coordinate caregiver placement and scheduling.
The Case Manager will work closely with patients, families, and caregivers to ensure each case is appropriately staffed and that day-to-day scheduling needs are handled efficiently. This position requires someone who can communicate effectively, think quickly, stay organized, and respond to last-minute staffing changes in a fast-paced environment.
Key Responsibilities
Manage an assigned caseload of patients and their ongoing caregiver needs.
Match caregivers with patients based on each family's specific needs, preferences, and scheduling requirements.
Coordinate schedules between patients, families, and caregivers.
Quickly re-staff cases when caregivers cancel or become unavailable.
Identify available caregivers and fill open shifts as efficiently as possible.
Maintain regular communication with patients, families, and caregivers.
Respond to scheduling changes, call-outs, and other time-sensitive issues.
Follow up with families to ensure appropriate caregiver coverage.
Build positive working relationships with caregivers and families.
Maintain accurate records of case assignments, schedules, changes, and communications.
Keep internal systems updated with current patient and caregiver information.
Work closely with internal teams to resolve staffing and scheduling challenges.
Escalate complex case issues to management when necessary.
Help ensure continuity of care by minimizing gaps in caregiver coverage.
Manage multiple cases and priorities while maintaining a high level of professionalism and responsiveness.
Qualifications
Previous experience in case management, home care, healthcare staffing, scheduling, coordination, or a related role preferred.
Strong verbal and written communication skills.
Excellent organizational and time-management abilities.
Ability to manage multiple patients, caregivers, and schedules simultaneously.
Strong problem-solving skills and ability to think quickly when unexpected staffing issues arise.
Comfortable making and receiving frequent phone calls.
Strong follow-up skills and attention to detail.
Ability to remain calm and professional in a fast-paced environment.
Dependable, proactive, and highly responsive.
Comfortable working in person in Williamsburg, Brooklyn.
Basic computer skills and ability to learn scheduling, CRM, or case-management systems.
