Social Worker

Position Summary
We are seeking a compassionate and experienced Social Worker to provide comprehensive case management services for clients with diverse needs. This role is responsible for developing and implementing holistic treatment plans, coordinating care, advocating for clients, and ensuring they receive the support and resources necessary to improve their overall well-being.
The ideal candidate is empathetic, organized, and experienced in working with individuals and families facing complex medical, social, emotional, and financial challenges.
Key Responsibilities
- Conduct comprehensive assessments of clients' physical, mental, emotional, social, and financial needs.
- Develop individualized treatment plans in collaboration with clients and the care team.
- Coordinate services and referrals with internal departments and external community resources.
- Advocate for clients to ensure access to appropriate programs, services, and benefits.
- Provide supportive counseling, guidance, crisis intervention, and ongoing emotional support.
- Collaborate with clients, families, healthcare providers, and community partners to ensure coordinated care.
- Monitor client progress and modify treatment plans as needed to support successful outcomes.
- Attend case review meetings, supervision sessions, and interdisciplinary team meetings.
- Maintain accurate and timely documentation, including assessments, treatment plans, progress notes, and case records.
- Participate in care planning and allocation meetings for assigned clients.
- Travel to clients' homes as needed to provide services and assessments.
Qualifications
- Master's degree in Social Work (MSW) or a related mental health field preferred.
- Minimum of three years of experience in social work, case management, or mental health services preferred.
- Strong assessment, counseling, and crisis intervention skills.
- Excellent organizational, communication, and documentation abilities.
- Ability to manage multiple cases while maintaining attention to detail.
- Proficiency in Microsoft Office, Microsoft Excel, Google Workspace, and electronic documentation systems.
- Ability to quickly learn case management and data tracking software.
- Familiarity with the Orthodox Jewish community is preferred.
- Ability to travel to clients' homes as needed.
